6. Proof of service of agenda/notice to the members of the Governing/General Body.
   
   
7. An up-to-date Minutes Book of the General Body**.
   
   
8. An up-to-date Membership Register**.
   
   
9. The Proposal which has been adopted by the General Body Meeting**.
   
   
10. An affidavit to the effect by the Secretary and the Chairman of the society that the provision of the section 12 of the S.R. Act. 1860 have been compiled with in the letter and spirit and also that there is no dispute in the Society*.
    
    
11. Comparative Statement of existing provisions and amended provisions*.
